NOTICE
** Site Under Maintenance
**

The Web site is under maintenance due to the apparent infiltration of malicious software sometime during the week of June 23, 2014 that may be automatically downloading malware on visitors' computers or devices.  It appears to be isolated to the Home page which is now temporarily replaced with this page until the situation is resolved.  Symptoms of infection include numerous popup windows, urgent alerts to download updated software, and redirected Web pages.   If you are NOT experiencing any of those, then there may be nothing to worry about.   The absence of the left side navigation column is an indication the problem has not yet been resolved.   Visitors are urged to run up-to-date virus and malware scans and/or reset their computer to a system restore point prior to June 23, 2014.

    Passport Services No Longer Available
    at the Waitsfield Town Office
    The Town of Waitsfield is disappointed to announce that the U.S. Department of State will no longer allow our office to accept passport applications for processing as of May 1, 2011.  The next closest acceptance facility is the Waterbury Post Office.  They accept passport applications Monday Friday, 10:00 2:30 and by appointment on Saturdays.  It is advised that anyone intending to get a passport call the Waterbury Post Office at 244-7318 to be sure he/she has all the necessary materials before going to Waterbury.  Also, it is helpful to have applications filled out beforehand.  Up-to-date forms can be completed and printed from the U.S. Department of State's Web site: www.travel.state.gov/passport/passport_1738.html.  The Waitsfield Town Office has blank forms, but it is unknown at this time if the U.S. Department of State will continue to send us forms as they are updated.

    FIRE PERMITS
    PUBLIC NOTICE
    The law in Vermont states that you must have a permit for outside burning.  Natural wood and brush are the only products allowed. Paper, cardboard, rubbish, tires, etc. are not permitted.   Please call at least one day in advance in the evening prior to the day of your burn.  Fires must be attended at all times no matter the size of the fire.

    TOWN of WAITSFIELD INVITATION FOR BIDS
    Waitsfield Intersection STP SRIN(34) & Waitsfield STP SRIN(37)
    DUE JULY 9, 2014

    Sealed bids from pre-qualified contractors shall be accepted until 2:00 pm, prevailing time on Wednesday, July 9 at the Waitsfield Town Office for construction of the project hereinafter described. Bid opening will occur immediately after the bid submittal deadline. The time of receiving and opening bids may be postponed due to emergencies or unforeseen conditions.

    Sealed BIDS shall be marked in the lower left hand corner: Bid Documents: Waitsfield Intersection STP SRIN(34) & Waitsfield STP SRIN(37).

    Each BID must be accompanied by a certified check payable to the Town of Waitsfield for five percent (5%) of the total amount of the BID. A BID bond may be used in lieu of a certified check.

    All bidders shall be on the current VTRANS Contract Administration pre-qualified list "Contractors List of Bike and/or Pedestrian Path and Road and Highway Construction Categories".

    LOCATION: Beginning at a point on VT Route 100 at approximately mile marker 3.795 and ending on Old County Road approximately 87.8' from the intersection with VT Route 100.

    TYPE OF CONSTRUCTION: Work to be performed under this project includes: The realignment of the VT Route 100 and Old County Road intersection, concrete sidewalk construction and associated work.

    CONTRACT COMPLETION DATE: The Contract shall be completed on or before October 15, 2014.

    OBTAINING PLANS: Plans may be obtained from the Blueprints Etc., 20 Farrell Street, South Burlington, VT 05403. Phone: 802-8654503 at a cost of $45.60 per set made payable to Blueprints Etc. Plans are not returnable.

    ENGINEERS ESTIMATE: For this Proposal the Engineers Estimate falls between $ 0 and $300,000.

    PREBID CONFERENCE: A non-mandatory pre-bid conference will be held for the project at 10:00 am on Thursday, June 19, 2014 at the General Wait House, 4061 Main Street, Waitsfield, VT 05673.

    STANDARD SPECIFICATIONS: This contract is governed by the Vermont Agency of Transportation ("VTrans") 2011 Standard Specifications for Construction.

    QUESTIONS: During the advertisement phase of this project all questions shall be made in writing and addressed solely to Greg Goyette, Stantec Consulting Services, Inc., greg.goyette@stantec.com.

    PREQUALIFICATION OF CONTRACTORS: All bidders on this project shall be on the Agency of Transportation's prequalified list under the categories Bike and/or Pedestrian Path and Road and Highway Construction or shall have submitted a complete prequalification application to the Agency of Transportation, Contract Administration, a minimum of 7 working days prior to the bid opening.     EQUAL EMPLOYMENT OPPORTUNITY (EEO) CERTIFICATION: Certification is required by the Equal Employment Opportunity regulations of the Secretary of labor (41 CFR 60-1.7(b) (1)) and must be submitted by bidders and proposed subcontractors only in connection with contracts and subcontracts which are subject to the equal opportunity clause.     NON-COLLUSION AFFIDAVIT: All bidders are required to execute a sworn statement, certifying that the bidder has not, either directly or indirectly, entered into any agreement, participated in any collusion, or otherwise taken any action in restraint of free competitive bidding in connection with such contract. See Appendix B for Debarment and Non-Collusion Affidavit (CA-91). This affidavit must be submitted with the bid.

    DEBARMENT AFFIDAVIT: All bidders are required to execute a sworn statement, certifying that the bidder has not within the last three (3) years, suspended, debarred, voluntarily excluded or determined ineligible by any Federal or State Agency; does not have a proposed suspension, debarment, voluntary exclusion or ineligibility determination pending; and has not been indicted, convicted or had civil judgment rendered against (it, him, her, them) by a court having jurisdiction in any matter involving fraud or official misconduct within the past three (3) years. See Appendix B for Debarment and Non-Collusion Affidavit (CA-91). This affidavit must be submitted with the bid.

    NON-DISCRIMINATION IN FEDERALLY ASSISTED CONTRACTS: The Town of Waitsfield hereby notifies all bidders that it will insure that any contract entered into pursuant to this advertisement, disadvantaged business enterprises will be afforded full opportunity to submit bids in response to this invitation and will not be discriminated against on the basis of race, color, religion, sex or national origin for an award. This is consistent with the Town's requirement to comply with provisions of Title VI.

    DAVIS BACON WAGE REQUIREMENTS: Bidders agree to abide by the Davis Bacon Wage Rate Schedule, which are appended to these Contract Documents.

    BUY AMERICA REQUIREMENTS: Buy America requirements of 23 CFR 635.410 are applicable to all Federal-aid construction projects. All steel or iron products permanently incorporated into Federal-aid projects, shall be products that have been entirely manufactured within the United States. All manufacturing processes of the steel or iron material, in a product, must occur within the United States to be considered of domestic origin. This includes process such as rolling, extruding, machining, bending, grinding, and drilling. The action of applying a coating to a material is deemed a manufacturing process subject to Buy America. Coating includes epoxy coating, galvanizing, painting, and any other coating that protects or enhances the value of the material.

    This requirement does not prevent a minimal use of foreign materials, provided the cost of foreign materials used does not exceed 0.1 percent of the total Contract price or $2,500, whichever is greater. The cost of foreign steel or iron is defined as its value delivered to the project.

    BROOK ROAD CULVERT REPLACEMENT
    The Town of Waitsfield is seeking bids for the installation of prefabricated concrete box culvert, head walls, and wing walls on Brook Road (TH #26), located a half mile up from Joslin Hill. This project is a concrete box measuring a 14' span X 7' rise X 70 LF and consists of the following:
    • (11) 6'-4' long precast sections.
    • (2) reinforced concrete cutoff walls, 18" wide X 18" high X 15'4" long.
    • (2) 8' long reinforced concrete tapered wing walls on inlet end of culvert fabricated with a 12" thick X 5' wide cantilevered footing.
    • (2) 8' long reinforced concrete wing-walls on inlet end of culvert fabricated with 12" thick X 5' wide cantilevered footing.
    • 32 LF separate wing wall footings, 24" wide X 36" high.
    The project must be completed by October 1, 2014. A mandatory pre-bid meeting will be held March 6, 2014, 10:00am, at the Town Office.     The Town of Waitsfield is seeking bids for the installation of a steel pipe, concrete saddle and large stones on Tremblay Road (TH #15) just below the Town Shed at 761 Tremblay Road. This project is a steel culvert that has a 7'11" span X 5'7" rise, and consists of the following:
    • (3) 26'6" steel culvert with bands.
    • (2) Concrete saddles, poured in place.
    • Type III stone.
    The project must be completed by October 1, 2014. A mandatory pre-bid meeting will be held March 6, 2014, 12:30Pm, at the site.
